The special easter egg dungeons only appear in the second and third games, as you suspected. Escape from the Pit is based on Jeff's very first game, Exile I. I suspect that he just didn't think about adding in something like this until the second game – and presumably he didn't want to slightly artificially add one in when he made the remakes. That's not to say that there aren't little eggs hidden all over the place, in all three games. Perhaps my favourite is this message in Crystal Souls: "This particular section of Avernum is quite buggy." You then get attacked. By bugs. Welcome to the boards!
