It's more the fact that it's a quest you get in the first town, and it doesn't go away until you've practically beaten the game. In fact, if I recall correctly, you basically have to do an about face on the way to the final confrontation if you want to finish it. That confuses a lot of people, because they finish the game, having forgotten about the quest, and it's still in their journal. Or they might realize it earlier in the game, but people tend to get confused when something as early as that isn't resolved. EDIT: Whoops.
