What is your screen resolution? If it's 800x600, you won't see it. Try Command+Tab to switch applications, though.

If you click on Settings from the main screen, unclick "Avernum takes up full screen"

You could also choose to play it in windowed mode, which will give it a Minimize button.

No, they're in different formats. Download the OS X version and email Jeff the new code, along with a note saying that you registered the Mac OS 9 versions. He should get back to you fairly quickly.
