You didn't HAVE to kill Sal. But it was a very real possibility. In any case, what you may or may not have done in previous games is not necessarily canon.

You only get that if you agree to his offer. You can still finish his quests just fine.

It was never supposed to be included in the first place and Jeff removed it for the Windows release.

There should be a special ending when you manage to piss off every faction.

Rawal isn't one of the five routes to victory. It does lead you to an ending, but hardly a preferable one.
