What are the four types of mushrooms we have to collect? I found the Diseased, Blighted, and Befouled. What is the fourth type, and where do I find it?

I know you can swap floors and terrain, but is it possible to get rid of items on the ground, or things like boxes, barrels, slime pools, blood stains, etc.?
